If you are paying by credit card or purchase order, you can mail, fax, or e-mail this completed registration form to WaterEC. If you are paying by check, please return the completed registration form with payment to the address below. Please complete a separate registration form for each individual registering to attend.
How to Complete the Registration Application:
This PDF form has been created so that you can type directly into it. Type into the form fields and use the tab key to move from field to field. Be sure to fill out all of the information and to include your e-mail address. Then print out the form and either fax, mail, or e-mail it to WaterEC. The information will not be saved if you are using Adobe Reader so be sure to print a copy for your records.
How to Submit Your Application:
Once completed, you may mail, fax, or e-mail the form to WaterEC. No cover sheet is needed when faxing. If you wish to e-mail the form, please include in the subject line "WaterEC Registration" followed by your name. A confirmation e-mail will be sent after your registration is received and input into our system, typically within 2 to 24 hours after submission. Weekend and/or holiday submissions will be completed on the next business day.
